Position Description
The Judge Group wants you or someone in your network to know about a Nurse Practitioner opportunity providing in-home and community-based healthcare services that are now available in the Maricopa County area.

Key Responsibilities
- Complete 25+ weekly in-home visits for assessments, diagnosis, medication management, and preventive care.
- Collaborate with healthcare teams, educate patients, and manage chronic conditions.
- Maintain accurate EMR documentation within 48 hours of visits.
- Ensure reliable transportation and participate in after-hours, on-call duties if needed.
- Adhere to healthcare regulations, company policies, and compliance requirements.
Qualifications
- Active Nurse Practitioner license with DEA and BLS certification.
- Certification as Family or Adult-Gerontology NP.
- Minimum 2 years of primary care experience preferred.
- Proficiency in EMR systems and strong clinical/communication skills.
- Valid driver’s license and car insurance with a clean driving record.
Preferred Experience
- Familiarity with HEDIS/STAR measures, home-based visits, and HCC documentation.
