Position Description

The Judge Group wants you or someone in your network to know about a Nurse Practitioner opportunity providing in-home and community-based healthcare services that are now available in the Maricopa County area.

Key Responsibilities

  • Complete 25+ weekly in-home visits for assessments, diagnosis, medication management, and preventive care.
  • Collaborate with healthcare teams, educate patients, and manage chronic conditions.
  • Maintain accurate EMR documentation within 48 hours of visits.
  • Ensure reliable transportation and participate in after-hours, on-call duties if needed.
  • Adhere to healthcare regulations, company policies, and compliance requirements.

Qualifications

  • Active Nurse Practitioner license with DEA and BLS certification.
  • Certification as Family or Adult-Gerontology NP.
  • Minimum 2 years of primary care experience preferred.
  • Proficiency in EMR systems and strong clinical/communication skills.
  • Valid driver’s license and car insurance with a clean driving record.

Preferred Experience

  • Familiarity with HEDIS/STAR measures, home-based visits, and HCC documentation.
