Dear Healthcare Provider:

My name is Eri Katagiri, and I am completing my Doctor of Nurse Practice (DNP) degree with the University of Arizona. I am requesting your participation in my DNP project survey, which aims to identify primary care providers’ perspectives on barriers to nutrition education in patients with type 2 diabetes mellitus.

Below is the link to a Qualtrics survey on the project topic. You will receive a reminder email one week from the initial survey date. The entire survey should take no more than 15 minutes of your time. Please read the attached Disclosure Form for more information on the potential risks and benefits of participating in this project.

Any primary care provider (e.g. medical doctor, nurse practitioner, or physician assistant) who meet the following criteria are asked to participate:

  • Experience practicing family medicine in southern Arizona within the last five years
  • Membership with or followers of this organization
  • Access to (and willingness to be contacted via) email
  • Ability to read and write in English

Your participation in this project is completely voluntary and confidential. By beginning the survey, you acknowledge that you have read the attached disclosure statement, are willing to participate in the survey, and understand that you are free to withdraw your participation at any time without penalty.

An Institutional Review Board responsible for human subjects research at The University of Arizona reviewed this research project and found it to be acceptable, according to applicable state and federal regulations and University policies designed to protect the rights and welfare of participants in research.
