Dear Colleague:

You are receiving this email because you have been identified as a medical professional that can prescribe medications to patients. As part of a grant through AHCCCS, the Center for Applied Behavioral Health Policy at Arizona State University is requesting that you complete a survey for medical professionals about their perceptions and behaviors related to prescribing Medication-Assisted Treatment to patients with Opioid Use Disorder (OUD).

The survey is anonymous, meaning there is no identifying information that would allow ASU to identify you or to associate your survey responses to you directly. Your participation in this survey is voluntary; you do not need to complete this survey if you do not wish to do so.

The survey should take no more than 15 minutes of you time. You may stop and save your survey at various points, allowing you to complete the survey in more than one sitting if you choose.
