We are looking for at least 4 Nurse Practitioners (either part-time or full-time) to do home health assessments in the Tucson area. I have included the job description below. If you know of anyone who is interested, please have them call me (my full contact information is at the bottom).
5 patients per day. Minimum 3 full days per week to be considered.
(Candidates Must be willing to travel and Must have EMR / IPAD skills)
FNP, Adult NP, or ACNP. New grads OK.
Providers conduct comprehensive, in-home Health/Annual Wellness Visits with Medicare patients as part of their health insurance benefits. The evaluations are conducted in a face-to-face manner where the member resides.
Primary Role and Responsibilities:
• Perform a comprehensive in-home medical evaluation of member and documents the findings
• Conducts review of medical records when available, interprets testing and exam results to develop a comprehensive report of diagnoses
• Documents follow-up care recommendations when appropriate
• Effectively represents the health plan in a positive manner to members and providers
• Properly identify and code medical conditions to the highest degree of specificity while complying with current coding requirements
• Reinforces to members the importance of staying current on covered preventive services
Compensation Details
· Salary range is anywhere between $100 - $120 per assessment depending upon experience.
